Unhappy LCD screen flickers on & off during gaming

My old LCD monitor is a Samsung SyncMaster 171v with an analog connection, running though a DVI adapter. My videocard is an evga nVidia GeForce 7600 GT videocard.

My problem is that in many of my videogames, the monitor often flickers off and on for several seconds. In some cases, the monitor "turns off" completely; the screen is all dark (yet the video game music is still playing) and the light turns from green to amber.

I am wondering if its my monitor (since its four years old already) and its refesh rate to blame, or is it my video card. I changed the refresh rate (from 75 to 60Hz) to see if the problem goes away. It went away in some games (such as HL2) but remains in others.
